• PART I - Needs to be filled in by applicant and a parent/guardian signature IS REQUIRED.
• PART II - Evidence of Age is completed by issuing official only (If applicant is not a PHS student, ID is required).
• PART III - If the applicant is a PHS student the School Nurse can complete the certificate of Physical Fitness if the students health records are up to date. If a non-PHS student this section should be filled out by the applicants physician.  Please note that the school nurse is not in the office during the summer months. If you are submitting a request during the summer, this section must be completed by your physician prior to submittal.
• PART IV - Is not necessary to have filled out.
• PART V - Is not necessary to have filled out.
• PART VI - Employment Certification is completed by the issuing official only.

When highlighted items have been completed, please return the papers to the Counseling Office to receive a permit.
